Andariel, Maiden of Anguish, is a Greater Lair Boss with a shared loot pool. To earn rewards, you will need to take her down while managing a series of mechanics. You can adjust the world tier in town to make the fight easier or more challenging. Higher Torment difficulty will reward you with better drops.
How to Summon Andariel
To open Andariel’s treasure after her defeat, you will need 3 Pincushioned Doll, which can drop from defeating The Beast in The Ice and Lord Zir.
Once you have the required materials, head to the Hanged Man’s Hall dungeon in Kehjistan. You can travel directly to the dungeon by right-clicking. Fight your way through, and at the altar at the end, use the altar to summon Andariel. You will need the materials to unlock his hoard once you defeated him.

Preparations
- Boost Your Survivability: Andariel uses a deadly mix of elemental and physical damage. If your character has not yet reached the defensive caps, use elixirs that increase Armor, Resistances, or Maximum Life. These can be crafted using materials gathered throughout the world.
- Play a Build You Trust: Bring a build with strong single-target damage and survivability. If unsure, check out our Boss Tier list for suggestions.
Boss Mechanics
The Maiden of Anguish has four taloned arms and will use attacks from each element, so make sure you have all resistances capped. Like most Lair bosses, she has a Stagger Bar; once it’s filled, it turns blue and will stun the boss temporarily, giving you a free damage window.
During the fight, in addition to her default attacks, Andariel has a few mechanics to look out for:
- Dash: Every now and then, Andariel will dash forward. Make sure to dodge it!

- Poison Projectiles: Multiple poison projectiles are launched in an arc circle in front of her; dodge them. Being hit by a projectile will result in a lot of poison damage over time.

- Lightning Balls: Andariel will sometimes spawn small lighting balls. These are easy to avoid, so they should not cause you too much trouble.

- Ground Seals: Avoid stepping into the seals on the ground when they spawn, as they will deal a lot of damage.

Andariel will disappear multiple times during the fight:
- The first time she disappears, Effigies of Anguish (pillars) will spawn in the arena. You will have to enter their shields to destroy them while also dodging a large Blood Beam, turning counterclockwise in the middle of the zone.


- The second time she disappears, you will have to destroy the effigies and dodge the blood beam, as well as Blood Marks on the ground, which will deal heavy damage if you step on them.

- Spinning Flame-Thrower Statues: At 50% health, Andariel’s arena spawns statues on the outskirts of the platform that launch a fire beam toward the center after the second disappearance of Andariel. Avoid the fire and move clockwise while also dodging the other mechanics.

Important: Any of these mechanics will give you a stack if you get hit. These stacks will affect how much damage you will take from the boss’s abilities. It is very important for your survival to avoid getting too many of these stacks from the very beginning of the fight.
Tips to Defeat Andariel
Andariel’s combat style has evolved significantly from her past incarnations. Her attacks are varied and can be deadly if not properly managed. Here are some things to consider, before facing Andariel:
- Be prepared! Before engaging Andariel, make sure you have sufficient survivability and, if you did not reach high defensive stat caps yet, use elixirs that increase Armor, Resistances, or Maximum Health. Elixirs can be crafted with components collected throughout the game. The Armor (Physical Resistance) cap is 85%, and Elemental Resistances cap out at 70%.
- Skill optimization: Adjust your skills to focus on single-target damage.
- Level up your glyphs at least up to Legendary Glyph status and progress in your paragon boards as much as possible.
- Try first in Torment 1 before you chose to face her in higher Torments. Andariel will scale to your World Difficulty.
- Mobility is important. Skills such as Teleport, Leap, or Dash, as well as plenty of Movement Speed and Dodge, will help you move out and away more quickly, as one of her attacks involves you having to dodge a lot of things at the same time.
Loot Table
To get your loot, you will need to use the required materials to open the reward chest or hoard after the fight. The first Unique dropped is guaranteed to come from the boss-specific loot table below. Any additional Uniques have a 50% chance to come from the general Unique pool instead.
If you are playing in a group, each additional party member increases the chance of an extra Unique dropping by 33%.

Belial Ambush Encounter
After defeating a Lair Boss, there is a small chance that Belial will ambush you. This unexpected encounter brings its own set of challenges, as Belial uses a mix of Blood and Poison magic that you will need to avoid.
Defeating this smaller form of Belial will grant you a second round of loot from the Lair Boss you just killed, without requiring any additional materials. He will also drop the Betrayer’s Husk, the item needed to challenge Belial’s Exalted Lair Boss form.
