Mad Monarch's Scepter

I'm using the Mad Monarch's Scepter as the weapon in my Kanai's Cube, and when I'm actually playing a game (i.e., not in town) above my skill bar there's an icon that says "Gathering Souls".  What does that mean?  Gathering souls to do what?

It will count up to 10, when it hits 10, then there is an AOE poison attack.

"After killing 10 enemies, you release a Poison Nova that deals 1050–1400% weapon damage as Poison to enemies within 30 yards."

Thank you!  I had absolutely no idea what it was doing.  That makes sense now.
