I was under the same assumption, but today my vault surprisingly included tier loot from Heroic Rygelon despite not having killed him yet on that character. Is it possible that the vault's rows and/or slots have different "rules"? For instance, could it be that if you were to earn 2 heroic kills you can earn gear in the vault from any heroic boss, however the chest/shoulder tokens will not show up in a dungeon slot until you've downed the respective bosses on the higher difficulties?
