What are Horrific Visions?
Horrific Visions are a limited-time game mode in World of Warcraft where players explore twisted versions of
Orgrimmar and Stormwind. Visions of what these cities might have become under N'Zoth's corruption.
You can complete them solo, with a companion, or in a group of up to 5 people. Horrific Visions provide
gear, and Displaced Corrupted Mementos that you can exchange for various rewards at Dornogal vendors.
When Will Horrific Visions Be Available?
Horrific Visions will be available starting May 20, 2025, on live servers in North America and on May 21, 2025, in Europe Currently, there is no end date indicated in the in-game calendar for the game mode.
What's New in Patch 11.1.5?
Horrific Visions were first added to the game during the Battle for Azeroth expansion. However, Blizzard have brought them back in Patch 11.1.5 and made some notable updates to the game mode. The following section offers a rundown of the most notable updates to the game mode:
- No keys are required to enter visions. You can run them as often as you like.
- The
Ashjra'kamas, Shroud of Resolve legendary cloak is not used in the updated game mode.
- A new currency called
Displaced Corrupted Mementos has replaced
Corrupted Mementos.
- Hourglass of Horrific Visions has effectively replaced the old Titanic Research Console.
- Tributes are passive buffs you unlock by completing certain objectives inside visions. They provide you with Sanity restoration on kills, and more.
- Soridormi is a new companion who can fill the role of a Tank, Healer, or DPS, and is not affected by Sanity.
- Blizzard added 3 new mask, so you can now attempt an 8-mask run for bragging rights!
- The item level of loot has been updated for Season 2. You will receive Hero-track gear from a 5-mask run when you complete all the objectives.
- New rewards have been added while all the old rewards remain!
- Corruption Effects return as Head Enchants in a very limited form.
Now that we have provided a comprehensive overview of the main differences, it is time to explain the mode in more detail!
Dornogal Horrific Vision Headquarters
Open the map of Dornogal by pressing the "M" key. From there, navigate to the Horrific Visions icon. This is your central hub for all Vision activities. This area includes the Hourglass of Horrific Visions, a talent-tree for unlocking perks during your runs, the vendors, and the NPC you must talk to to enter Horrific Visions!
Hourglass of Horrific Visions
Hourglass of Horrific Visions acts as a talent tree for Horrific Visions. Every week, players can unlock
a limited number of permanent perks that will gradually reduce your Sanity loss during your runs, allowing you to take on more objectives
during a single run. Every perk costs an increasing number of Displaced Corrupted Mementos that you exchange for 1
Echo of N'zoth
at Augermu. You will learn more about the talent system in our separate guide!
How to Enter Horrific Visions?
Upon completing the intro questlines, you can interact with the Portal to Horrific Visions in Dornogal to queue up for the game mode. When you start a Horrific Vision, you will be placed inside a corrupted version of either Stormwind or Orgrimmar (changes weekly).
Horrific Visions Safe Room
At the beginning of your vision, you will be teleported to a safe room where you can:
- Activate the Masks for your run (we will cover this in more detail later);
- Check your Tribute progress;
- Hire an NPC companion for your run that can fill the role of a Tank/Healer/DPS.
To begin your run, talk to the Image of Wrathion.
Horrific Vision Basics
Your main goal is simple. Stay alive, keep your Sanity from running out, and defeat the final boss, either Alleria in Stormwind or Thrall in Orgrimmar, while completing as many side objectives as possible to improve your rewards.
Sanity Bar
When you start the run, a big purple Sanity Bar appears on your screen. It slowly drains over time and when you take damage from certain abilities. If it hits 0, you are teleported out of the vision and your run ends.
Sanity works as a built-in timer for all vision types. Players start with a set amount of Sanity that can be increased by Hourglass talents. The rate at which it drains depends on the area you are currently in, any abilities you get hit by, and other factors like active debuffs.
You can restore your Sanity by using Sanity Restoration Orbs and by killing Elites if you have unlocked the Hourglass perk. Some talents and bonuses will also reduce the amount of Sanity you lose over time.

Objectives and Zones in Horrific Visions
When you enter a Horrific Vision, your main objective is to defeat the final boss, Alleria (in Stormwind) or Thrall (in Orgrimmar).
You can choose to go straight for the boss at the start, but your rewards will be much lower. To earn better loot and more
Displaced Corrupted Mementos, you should complete side objectives first!.
Each Vision is divided into multiple subzones, each with its own set of challenges called Madnesses that rotate weekly. In these subzones, you must complete various objectives and defeat mini-bosses. Clearing these side zones increases the difficulty of the final boss, giving them new abilities and making the fight tougher, but also improving the rewards you receive at the end of your run.
When you fully clear a side zone, a teleporter will appear, allowing you to quickly travel back tot he starting area of the vision.
Zone Types
Each horrific Vision has 3 types of zones, each one harder than the last:
- Tainted Zones (Tier 1): These zones have the lowest Sanity drain, around 6 Sanity per second.
- Corrupted Zones (Tier 2): Medium difficulty zones with a Sanity loss of 8 per second.
- Lost Zones (Tier 3): The hardest areas with 10 Sanity drained per second.
As you move deeper into the vision, both the enemies and the Madness effects grow stronger!
Madness Debuffs
Every subzone applies a random Madness deuff that makes surviving more difficult. Madnesses vary from mild inconveniences in Tainted Zones to major hazards in Lost zones. You may face hallucinations, disorientation, Sanity-draining traps, and other hazards. The currently active Madnesses will be indicated in the top center of your screen.

How a Horrific Vision Ends
Your run ends in one of three ways:
- You die with no ally to resurrect you (unless you have the resurrection perk unlocked via the Hourglass);
- Your Sanity bar drops to 0, kicking you out of the vision;
- You defeat the main boss (Alleria or Thrall), completing the run.
Your First Few Runs
During the first week of Horrific Visions, you will not be able to complete more than a few of the side objectives before
your Sanity reaches 0. This is completely normal and expected, as you do not have access to the perks granted by the hourglass.
Use these runs to farm Displaced Corrupted Mementos to buy Vision Upgrades to prolong your stay in the visions!

Horrific Vision Potions
Both Horrific Visions have a potion mechanic where players get different buffs depending on the color of the potion they drink. The potion effects are randomized each time you enter, but we will have a guide to help you figure out which potion color you should not drink to avoid bad debuffs in the near future.
Faceless Masks
Faceless Masks are special challenge modifiers you can activate before starting a Horrific Vision.
Each mask makes the vision significantly harder by adding a major penalty, but in return, you earn
better gear and more Displaced Corrupted Mementos. You can equip up to 8 masks at once
in Patch 11.1.5.

Tributes
Tributes are powerful passive bonuses unlocked permanently by achieving certain milestones inside Horrific Visons. You unlock them automatically without spending any currency. Once you unlock a Tribute, it works on all your characters. You can view your current Tribute progress in the Horrific Vision safe room before you start your run.
Corrupted Helm Enchants
New in Patch 11.1.5 are Corrupted Helm Enchants. These bring back powerful effects from Battle for Azeroth's Corruption System like Twilight Devastation in a limited way without the old curses/drawbacks.
You unlock them by completing a short quest and collecting some items that drop at the end of your visions.

Rewards from Horrific Visions
Horrific Visions offer a wide range of rewards, from achievements and titles to hidden secrets waiting to be uncovered. We will have a full guide to all Horrific Vision rewards available soon!
End Chest Rewards and Loot System
Completing Horrific Visions rewards you with chests at the end of your run. The more side objectives you clear and the more Masks you use, the better your rewards will be. You can find a variety of collectibles inside these chests, including pets, mounts, and gear.
The item level of gear you receive depends on how much of the vision you cleared and how many Masks you had active.
How the Horrific Vision Loot Works
The main goal in a Horrific Vision is to defeat the final boss and complete all side objectives. Using Faceless Masks increases both the challenge and the quality of loot. You can earn loot once per week per character and per Mask difficulty. This means you can get rewards from different Mask levels. For example, completing a full 4 Mask and a 5 Mask run will grant you 2 pieces of Hero-track gear on the same character.
What Gear Can You Get?
Here are the exact item levels of gear you will receive from the chests at the end of your run. Keep in mind that loot caps at 5 Masks for maximum gear quality (Hero 658).

Bonus Rewards: Gear Upgrade Crests
In addition to gear, each run also gives you Undermine Crests used for The War Within Gear Upgrade System. The type of crest you get depends on how many Faceless Masks you have active during your run! Keep in mind that Blizzard plans to lift the Crest cap on May 13 before Horrific Visions become available on live servers!
